Some contributions don't add the fee in `civicrm_financial_trxn`

Most of the time it seems a payment will add the contribution as well as a separate entry for the fee in the civicrm_financial_trxn table. On some contributions, the fee is not added for some reason, which makes financial batches inconsistent (at least as has been reported to me). Has anyone else seen this? Here's a shot that tries to show what feels like an inconsistency:
