Added address custom fields to certain templates

Hi @sluc23, this merge add address custom fields to certain templates:

  • Case Matrix Plus
  • Contact Matrix Plus
  • Contribution Matrix Plus
  • Member Matrix Plus
  • Member Summary Plus

I think now better not to add it in the birthday and activity template because I think these templates need a previous revision.
