Use "new" hook invocation

Hook invocation change several years ago - see https://github.com/civicrm/civicrm-core/pull/17995

This updates to the new syntax, and also drops support for CiviCRM < 4.5 (!)

@jaapjansma Do you have any code using those hooks? I think this is right but have not tested it.
