Upgrades to work with 4.5

colemanw requested to merge github/fork/colemanw/upgrade into master

This will break compatibility with 4.4 so we probably want to create a separate branch for the project. CiviVolunteer is doing it like: 4.4-1.x 4.5-1.x etc. Or to keep it simple, just put out a new release and remove the 4.4 compatibility tag.
