Add support for multiple option fields which have a Membership Type assigned

Add support for multiple option fields which have a Membership Type assigned. Does not currently work with checkboxes and multi-select option fields which allow the user to select more than one membership type in the same transaction.

This is an edge case where an organisation allows a single contact to have more than one membership type at a time.
