Don't redeclare variable for no reason

DaveD requested to merge DaveD/cdntaxreceipts:report-form into master

In https://github.com/civicrm/civicrm-core/pull/23912 the scoping of the form var was revised. For some reason it's redeclared here, possibly from some old copy/paste.
