#46 - Add Location Issued field

DaveD requested to merge DaveD/cdntaxreceipts:location-issued2 into master

Longstanding missing field.

Alternate version where there is only a setting on the settings page, and the admin has to change it before running receipts if there are multiple locations. On upgrade there is no default, so it will prevent receipting until it gets set.

Also the field is available in the Receipts Issued CiviReport.
