Add num_terms to CreateOrUpdateMembership actions

Francis (Agileware) requested to merge agilewarefj/action-provider:CVAP-47-membership_num_terms into master

Change allows multiple terms to be added to an existing membership based on calculation rules in the system. Before this change, only 1 term is available without complicated procedures earlier in a form processor to calculate the correct end date; and start date for renewal of expired memberships is borderline impossible.
