Add cross-references #254

This is a partial fix for https://github.com/civicrm/civicrm-user-guide/issues/254

I've worked on the following sections: Case Management, SMS, Initial set up, Survey, Reporting

Also did a little bit of minor tidying up and made image links absolute references
