In wordpress, the csv-import-helper page is blank

This is in civi 4.7.14 and wordpress 4.7.2. The extension is 1.1.3-beta. After installation, the new menu item appears in the menu, but the csv-import-helper page is blank. There are no errors in the browser console. However there is an error in the log - see it on pastebin.
