Form Builder: provide easy way to create 'Add Certification' button

The Certification listing provides view/edit/delete links for existing Certifications. Want to be able to easily add a button for Add Certification.

Currently, a button was added by manually editing the afform html but does not do anything. The links are already known via the xml so seems like it should be fairly simple.
