contribution ThankYou.tpl.php notice errors on dmaster, contribution total amount $0

Scenario 1: On dmaster, with default value for mug premium ($12.50), on live Donation page, choose $50 donation, ask for mug, complete credit card info, no honoree, no title for donor, then on thank you page you get errors below.

Simplifying scenario 2: on dmaster Donate page, leave default $10 amount, no premium, fill in required fields for Billing Name and Address as shown, credit card info, then on thank you page get errors below. Note that Title field is not provided on page, and honoree sections not on page. Need to change template to make them optional is my guess.

BUG: in both cases, it shows total paid was $0.00.
